When OpenAI alone stops being enough

When you don't have systems for Frontier to orchestrate.

Frontier's value is orchestrating across modern, well-API'd systems-of-record. If your hospital is on a 15-year-old Epic install with no internal data warehouse, there's nothing to orchestrate yet — you need the application first. GEIST ships the application; Frontier sits on top later if you grow into it.
